In a serious safety lapse at the Metro Line 5 (Thane Bhiwandi Kalyan), an iron tie rod fell from an overhead structure onto an autorickshaw, causing a head injury to a passenger at Bhiwandi on Tuesday August 5th. The Mumbai Metropolitan Region Development Authority (MMRDA) has imposed a Rs 50 lakh penalty on the civil contractor Afcons for the incident.
Passenger Grievously Injured
The rod, part of a temporary support system installed two months ago for the deck slab, fell sparking concern over safety protocols at the site. The injured passenger was immediately rushed to the hospital, with safety heads from both Afcons and general consultant Systra-CEG-Systra India attending and ensuring prompt medical care.
MMRDA officials confirmed that Afcons has been directed to bear all medical expenses and extend full support to the injured.

MMRDA Charges Fine Of ₹50 Lakh On Contractor
In addition to the Rs 50 lakh fine on the contractor, MMRDA has also penalised Systra-CEG-Systra (India) Rs 5 lakh for supervisory lapses. An independent inquiry has been initiated under the Chief Safety Manager of Metro Line 2B’s general consultant, AICA, to investigate the failure and suggest corrective actions.

"Public safety is our highest priority. Strict accountability measures have been initiated, and further actions will follow based on the inquiry's findings," an MMRDA official stated.
